IT Declaration Application is not gettng open

Hi ESS Experts,

I have configuration and maintained application paramter details for IT Declaration Application in LPD_CUST for EHP7. I can see intial screen of IT declaration application but when I select Create button, system shows

Kindly provide your suggestion.
