How to add IT22(Education) to ESS via Process and Forms

Facing issue when I am trying to add subtype for IT22 using Process and Forms. Can any one please guide how exactly to see Subtype(SYBTY ) for IT22 in Adobe Form as dropdown list. Here for PA22 subtype SLART represents SUBTY.
